About the role:

We are currently looking to a hire an experienced Case Manager, this role will work with NSW Juvenile Justice and with young people who are in contact with the juvenile justice system. It also works with the significant people in the life of the young person where there are relationship difficulties and conflict. It provides short-term therapeutic intervention which focuses on enabling the young person and family to establish life goals and improve their relationship with each other.

Key responsibilities (not limited to):

Ensure that program delivery is strengths based and flexible in terms of session times, length, location and language used, so as to accommodate individual participant needs.
Ensure group programs are evidence based and meet client needs by ongoing review and evaluation
Ensures that all parties are aware of the case plan by providing a plan that is clearly defined and with achievable outcomes in collaboration with the young person and Juvenile Justice.
Ensure a full client caseload is maintained by working within funding guideline and program targets.
Ensure an efficient and effective client service is delivered by building linkages with key community partners, working collaboratively with relevant parties and applying early intervention strategies as required.
Ensure documentation meets organisational, legislative and funding requirements by completing and maintaining appropriate client documentation and statistical information.

Skills, knowledge and experience needed:

Tertiary qualifications in Social Work, Counselling, Community welfare or equivalent
Experience in working with disadvantaged groups including families, parents, children and young people
Experience in counselling, mediation and high level client casework
Computer literacy
Well-developed written and verbal communication skills including the ability to network and establish positive relationships
Ability to support and promote the Christian Mission and Values of Anglicare
